1) Contact Your State Department
You should first contact your State Department that deals with licensing for your type of agency that you want to start. You should obtain a "License application ". You should also get a copy of any necessary licensing laws or regulations.

2) Documents Needed for Licensing
The license application pack or state website will have all the requirements for starting your agency. It will also request a list of documents to be submitted with your filled in application or needed at survey (depends on your state) - called "Policies and procedures". And Job Descriptions.
